2008-09 Sports Physicals

Athletes must have a physical on file with their coach to participate in any
 sport for the 2008-2009 season.

 This includes any summer practices that take place.

You may find the physical form for 10th, 11th, and 12th grades here:

 You will need to print both pages.

 For athletes in 9th grade, a specific clearance form found on the Tennessee
 Department of Health website must be used.  This is the only form required
 for 9th grade athletes.  http://health.state.tn.us/sportsphysical
